Explorer Program Application

If the applicant is under 18, a parent or guardian must give permission for the applicant to participate in the Jefferson County Sheriff's Office Explorer Program and must sign the printed application. Enter the parent or guardian's name below; they will sign the printed form.

The printed application includes the following release, completed with the name and date of birth you enter below:

I, (your name) __________, Date of Birth __________ do hereby authorize the Jefferson County Sheriff's Office Explorer Program to have access to any records your agency may have concerning me, my school records, criminal records, driving record, juvenile criminal record and employment records.

I have also included a photocopy of my government issued birth certificate and photo identification with this application.

Include the following documents with your printed application:

• Certified copy of birth certificate

• Copy of Social Security card

• Copy of valid Colorado driver's license or ID

• Copy of current high school transcript

• Copy of high school diploma or GED (if applicable)

• Official college transcripts (if applicable)

• Copy of special license/certificate (if applicable)
